How Our Theory of Change Comes to Life

From understanding the challenge to measuring impact, these ten steps show how our work creates lasting change.

Listening to communities

1. Understand the Context

We start by listening to communities, health workers, and partners to map the real challenges on the ground.

Defining the problem

2. Define the Problem

Together we identify the core barriers that block access to quality, equitable health services.

Mapping actors and systems

3. Map Actors & Systems

We look at who is involved in the system and how data, tools, and resources currently flow.

Inputs and resources

4. Mobilize Inputs

Funding, skills, partnerships, and technology are aligned to support the change we want to see.

Activities in the field

5. Deliver Key Activities

We co-design tools, deliver training, and support implementation with clinics and community programs.

Outputs and services

6. Create Tangible Outputs

Digital tools, guidelines, and support services are in place and actively used by health teams.

Short-term outcomes

7. Achieve Early Outcomes

Health workers gain confidence, data quality improves, and teams coordinate more effectively.

System-level outcomes

8. Shift Systems

Decision-makers use evidence, policies adapt, and services become more responsive and reliable.

Long-term impact

9. Reach Long-Term Impact

Communities experience better health outcomes and reduced inequalities in access to care.

Learning and feedback

10. Learn & Adapt

Data and feedback loop back into the model so we can refine our approach and strengthen impact over time.

Our Theory of Change | What we Believe

We believe that when health workers have the right tools, data, and support, the quality of care in Liberia improves for everyone.

Health Tech Liberia – Our Theory of Change

We use technology, data, and local partnerships to strengthen how health services are delivered across Liberia.

We start by listening to health workers and communities to understand real challenges in clinics and everyday care.

We then co-design simple, practical digital tools and support teams to use them in their daily work.

Over time, better data and stronger systems lead to more timely, reliable, and equitable healthcare for everyone.
